Hardly a day goes by when an article does not appear somewhere in the trade media and even the general news media that recounts some error leading to a tragic patient outcome - see the link to the Washington Post story in the "News Headlines" section below. Those of us in Healthcare know how hard we work to make sure errors don't happen. But, they still do. I suppose, to be honest, eliminating errors 100% of the time is impossible. To error is human. But when it comes to healthcare, a small error can have big time consequence for a human life. So we strive to reach 100% and can never stop striving for delivering perfection.
